A new understanding of health related empowerment in the context of an active and healthy ageing

Abstract

BackgroundRecently, several initiatives have focused on how to create true person-centred health services. This calls for a new understanding of health-related empowerment in relation to people living with one or more chronic conditions.We report on a Delphi investigation among participants in the European Innovation Partnership on Active and Healthy Ageing that has led to a new understanding of health-related empowerment.MethodsThe Delphi process was conducted in three sequential rounds. In the first round, we presented a suggested first version for a definition of health-related empowerment divided into nine statements. One hundred and twenty-two experts were then asked if they agreed or not with each individual statement, and in the case they disagreed, to state the reasons for their disagreement.After revisions, the experts who had replied to the first version were asked again, if they agreed or not with each individual statement of the second version and to elaborate on disagreements.Finally, in the third round the experts were asked to provide comments to the final proposed definition in general and not by each statement.ResultsA total of 33 experts responded to the first version. The following revision included a merging of two statements, and the addition of health literacy as part of the understanding. The second version was sent out to the 33 experts and a total of 19 experts commented with moderate consensus. Changes included removal of self-esteem and change of self-confidence to confidence.Third version was sent out to all 122 experts with 16 respondents. Strong consensus was obtained for this third version, and is with one minor change presented as the final version.ConclusionWe propose a new understanding of the concept health-related empowerment, by focusing on the individual as a co-manager with freedom to choose and focus on their own well-being.

Bibliometric impact. Analysis of the contribution and dissemination channel

From the perspective of influence or social adoption, and based on metrics associated with mentions and interactions provided by agencies specializing in calculating the so-called "Alternative or Social Metrics," we can highlight as of 2026-04-25:

  • The use, from an academic perspective evidenced by the Altmetric agency indicator referring to aggregations made by the personal bibliographic manager Mendeley, gives us a total of: 160.
  • The use of this contribution in bookmarks, code forks, additions to favorite lists for recurrent reading, as well as general views, indicates that someone is using the publication as a basis for their current work. This may be a notable indicator of future more formal and academic citations. This claim is supported by the result of the "Capture" indicator, which yields a total of: 160 (PlumX).

With a more dissemination-oriented intent and targeting more general audiences, we can observe other more global scores such as:

Project objectives

Los objetivos perseguidos en esta aportación se centran en avanzar en la comprensión del empoderamiento relacionado con la salud en personas con condiciones crónicas. Se pretende analizar la definición inicial de empoderamiento en salud mediante un proceso Delphi con expertos europeos. Evaluar y revisar las afirmaciones propuestas para alcanzar un consenso sobre los elementos clave del empoderamiento. Determinar la inclusión de aspectos como la alfabetización en salud y ajustar términos como la confianza. Caracterizar el empoderamiento como un proceso que sitúa al individuo como co-gestor de su bienestar con libertad de elección. Finalmente, consolidar una definición consensuada que refleje esta nueva perspectiva.

Most relevant results

El estudio presenta una nueva comprensión del empoderamiento relacionado con la salud en el contexto del envejecimiento activo y saludable, basada en un proceso Delphi con expertos. Los resultados más relevantes son: 1) en la primera ronda, 33 expertos evaluaron una definición inicial dividida en nueve afirmaciones; 2) tras la revisión, se fusionaron dos afirmaciones y se incorporó la alfabetización en salud; 3) en la segunda ronda, 19 expertos mostraron consenso moderado, eliminando la autoestima y modificando la autoconfianza a confianza; 4) en la tercera ronda, 16 expertos alcanzaron un fuerte consenso sobre la versión final, con una pequeña modificación; 5) se propone un concepto centrado en el individuo como co-gestor con libertad para elegir y enfocarse en su bienestar.
